To register at Gosport Medical Centre please pick up a new patient registration pack from the surgery.
This will need to be fully completed and returned to the surgery by the person registering between 1200-1400 or after 1700 with two forms of identification (one needs to be photographic, such as a passport or driving licence and the other a letter received within the last six months that shows your current address, such as a utility bill.)
When you bring your form back you will be required to have your blood pressure and weight taken, there is a BP machine in the waiting room for this. You will then need to book a 10 minute telephone appointment with one of our Health Care Assistants.
Your new patient registration can take a couple of weeks to process. You will not be registered at Gosport Medical Centre until after your appointment.
If you have regular repeat prescriptions please request at least a month’s supply from your current surgery. This will enable us to complete your registration and receive your medication records before issuing repeat medications.
Please note that our Receptionists are unable to book GP appointments directly and all requests for an appointment are triaged by our clinical team before an appointment is given. The Receptionist has been authorised by our GPs to ask certain questions to ensure that the clinical team can triage your request appropriately. All our staff are bound by the surgery's confidentiality agreement.
