TRANSFER OF PATIENTS FROM BROCKHURST MEDICAL CENTRE
As you may be aware Brockhurst Medical Centre has had to close earlier than expected. Fareham and Gosport CCG and all local surgeries have been working hard to ensure that everyone still has access to medical services and that your care is transferred as smoothly as possible.
If you are registered with Brockhurst Medical Centre please do not worry, your medical information is due to be merged with your new practice over the weekend. You do not need to take any steps to register with a new surgery.
Patients from Brockhurst Medical Centre will be receiving a letter letting them know which GP surgery they will be registered with.
IF OVER THE WEEKEND YOU HAVE AN URGENT MEDICAL PROBLEM THAT CANNOT WAIT UNTIL MONDAY PLEASE CALL NHS111 OR FOR ANY LIFE THREATENING ISSUES PLEASE CALL 999.

Welcome to our practice!
To make your transition easier we have attached an information sheet on how to best access our services. We will also be sending out this information via text, email or post depending on the contact details we have for you. It is very important therefore that we are kept up to date with any change of your details.
Your online access will have been suspended and you will now need to link this with our surgery. We have attached instructions on how to do this. Please do not do this before Monday as your records will not be transferred until then.
Due to the volume of calls that we are expecting to receive, please can we ask that you do not contact us in the first few days unless you have an urgent medical problem that needs treatment.
To ensure we adhere to the social distancing rules we have to limit the amount of patients that come into the surgery. Therefore please do not come to the surgery unless you have a pre-booked appointment.
